Dealer Application
Form Instructions:
1. Select the hand tool from the toolbar, and the Cursor will turn into a hand.
2. Position the Cursor and click inside the first form field. The Cursor will change to an insertion point.
3. Enter the appropriate text in the form field, and Press TAB to move to the next form field.
4. You can Tab over the checkboxes, and Press Enter to place a checkmark in them. Alternatively, you can just click on them with the cursor.
5. If your version of Adobe Acrobat allows you to save the form AND the data, save it and email it back to us as an attachment at sales@misco.com.
6. If you cannot save a copy with the data, you will need to fill it out, print it with the data, and then fax it to us at 216-831-1195. You may also print out the document and fill it out by hand.
